- ePatient + Chairside forms: $79 (per month)
Your account comes with 10 staff users. Over ten staff users is an additional $10/per month.
Each account comes with 30 day money-back guarantee. If at the end of 30 days you do not wish to continue, you can unsubscribe or notify your Dental Symphony Representative and be refunded your first payment (This refund does not inlude the Setup Fee).
Setup Fee: We charge a one-time $99 setup fee for each location. This helps ensure a speedy installation and account customization to get you up and running. All future support, customization, or installations on other computers at that location is included in your subscription fee. This one-time setup fee is non-refundable.
Multiple locations: If your office has multiple locations, you will need unique accounts for each location because of different patient databases, different staff, and potentially different customizations.
If you need to use one account at more than one location, your subscription rate is discounted according to the prices below.
- 2-5 locations: each account is 29% off ($49).
- 6-10 locations: each account is 44% off ($39).
